Classroom Walk-Through Forms Using Google Forms

We have received many emails in regards to developing a Google Form to record data for school improvement plans. Below is a copy of the actual form that was implemented at Jefferson Middle School. Administrators access this form on an Apple Ipad to gather data based on classroom observations. This form was designed with emphases on data entry by point and click, rather than typing information. This provides the user with a quick and easy form of data entry. Below is a screen capture of a Google Spreadsheet that populates when data is entered into the form. Development procedures and screen captures are available below.

When data is inputted, the information populates a Google Spreadsheet that is similar to the one at the left. Data can then be sorted based on specified parameters.

Here is an example of the form in development. Google Forms allow the user to select the question type. Drop-down menus were developed using the "choose from a list" option. Other question types also included text, paragraph text, and the checkbox option. Question types were based on targeted outcomes to suit our specific needs.
